Crise Adrenal Aguda Em Paciente Com Síndrome de Schmidt: Um Relato de Caso

Resumo: A Síndrome Poliglandular Autoimune (SPA) pode ser definida comoa disfunção de ao menos duas glândulas endócrinas, estandoassociada, ou não, ao comprometimento de outros órgãos nãoendócrinos. Tais síndromes podem ser subdivididas em quatrosubtipos, sendo a SPA 2 a mais comum. Ela engloba a associaçãoentre a doença de Addison, disfunção tireoidiana autoimune ediabetes mellitus do tipo 1. Tais patologias estão presentes emdiferentes combinações, podendo também estar associadas aoutros distúrbios menores. A Síndrome de Schmidt consiste naassociação entre a doença de Addison e a disfunção tireoidianaautoimune. Ela representa uma das apresentações da SPA do tipo2. Neste trabalho, é abordado o caso de uma paciente do sexofeminino acometida pela síndrome de Schmidt, já em seguimentoambulatorial, que foi internada pela presença de náuseas, vômitos,dor/defesa abdominal, síncope e alterações hidroeletrolíticas. Foiaventado o diagnóstico de crise adrenal aguda, um quadroconsiderado como emergência médica. Desse modo, foi realizadotratamento composto por reposição volêmica vigorosa associada àadministração de hidrocortisona e fludocortisona, com melhoraclínica. Com este caso, pretende-se fornecer informações para aclasse médica sobre a enfermidade em questão e suasparticularidades. Título em Inglês: Acute Adrenal Crisis In a Patient With Schmidt Syndrome: a Case Report

Resumo em Inglês: Autoimmune polyglandular syndrome (APS) can be defined as thedysfunction of at least two endocrine glands associated, or not, withthe involvement of other non-endocrine organs. Such syndromescan be subdivided into 4 subtypes, with APS 2 being the mostcommon. It encompasses the association between Addison'sdisease, autoimmune thyroid dysfunction and type 1 diabetesmellitus. Such pathologies are present in different combinations, andmay also be associated with other minor disorders. Schmidt'sSyndrome is the association between Addison's disease andautoimmune thyroid dysfunction. It represents one of the type 2 APSpresentations. In this work, the case of a female patient withSchmidt's syndrome, already in outpatient follow-up, who washospitalized due to the presence of nausea, vomiting, abdominal pain/defence, syncope and hydroelectrolytic alterations, isapproached. The diagnosis of acute adrenal crisis was suggested, acondition considered as a medical emergency. Thus, a treatmentconsisting of vigorous volume replacement associated with theadministration of hydrocortisone and fludocortisone was performed,with clinical improvement. This case is intended to provideinformation to the medical profession about the disease in questionand its particularities.
